1.)    Parent FSA ID (www.fsaid.ed.gov)

2.)    Student FSA ID (www.fsaid.ed.gov)

3.)    Parent’s 2016 federal and state tax return

a.      For non-tax filers, documentation of 2016 income

4.)    Student’s 2016 federal and state tax return

a.      For non-tax filers, documentation of 2016 income

5.)    Student SSN and date of birth

6.)    Parent SSN and date of birth

7.)    Dates of birth for any other members of the household

8.)    Marriage, Divorce or separation date of parents
